Vision Coverage (Optional Add-On Rider)
The Vision Rider adds eye care coverage to your base dental plan. Here is what it covers:

One complete eye exam per year — including refraction — covered at 60% year 1, 70% year 2, and 80% thereafter.
👓 Eyeglass Lenses
Single, bifocal, trifocal, and progressive lenses — one set per year. Same coverage percentages as the eye exam — growing each year.
🖼️ Eyeglass Frames
Up to $200 per year toward eyeglass frames. Combined with lens coverage, this covers a substantial portion of a complete pair of glasses annually.
📦 Contact Lenses
Contact lens coverage in lieu of eyeglasses — up to $200 per year. Contact lens fitting fee also covered up to $15 per year.
🔮 Lens Upgrades
Anti-reflective coating covered up to $45/year. Polycarbonate lenses covered up to $40/year. Useful additions for those who need specialty lenses.
Hearing Coverage (Optional Add-On Rider)

How the Plan Works — A Real Example
Maria, age 52, enrolled in the $1,000 maximum plan with a $100 deductible. She visits a Careington network dentist in Harlingen for her regular checkup. The dentist discovers she needs a filling.
Dental exam: Provider charges $150. In-network contracted rate is $35. The plan covers 100% of preventive services — Maria pays $0 for her exam.
Filling: Provider charges $275. In-network contracted rate is $99. After her $100 deductible is applied, the plan covers 65% of the remaining in-network cost. Maria pays approximately $35 out of pocket for the filling.
Total Maria pays for both services: approximately $35 — compared to $425 without any coverage at all. Her annual maximum of $1,000 remains largely intact for any additional dental needs throughout the year.
